Acetyl-L-Carnitine — The Mitochondrial Transport Factor

Acetyl-L-carnitine (ALCAR) is a naturally occurring amino acid derivative that transports long-chain fatty acids into mitochondria for beta-oxidation. The acetyl group also serves as a substrate for acetylcholine synthesis, making ALCAR uniquely positioned at the intersection of mitochondrial energy metabolism and cholinergic neurotransmission. ALCAR levels decline with age.

Mechanism of Action

ALCAR facilitates the transport of acyl groups across the inner mitochondrial membrane via the carnitine shuttle, enabling fatty acid oxidation. The acetyl group is released as acetyl-CoA (entering the TCA cycle) or used for acetylcholine synthesis. ALCAR also upregulates nerve growth factor (NGF) receptors, has antioxidant properties, and restores mitochondrial membrane potential in aged cells.

Human Trial Evidence

A 2003 PNAS study showed ALCAR + alpha-lipoic acid reversed mitochondrial decay and cognitive decline in aged rats. Multiple RCTs show ALCAR (1,500–3,000 mg/day) improves cognitive function in mild cognitive impairment and Alzheimer's disease. A 2004 meta-analysis of 21 RCTs confirmed significant cognitive benefits in Alzheimer's patients.

Dosing Protocol

500–2,000 mg/day. Best taken in the morning (mild stimulating effect). Can be split into two doses. ALCAR is more bioavailable than L-carnitine and crosses the blood-brain barrier. Often combined with alpha-lipoic acid (synergistic for mitochondrial function). Effects on energy and cognition typically noticed after 2–4 weeks.

Safety & Contraindications

Well-tolerated. May cause fishy body odour (trimethylamine production by gut bacteria) — less common than with L-carnitine. Mild GI effects at high doses. May increase TMAO levels (cardiovascular concern in some individuals). Stimulating effect — avoid in the evening. Caution with thyroid medication.
