Bilberry extract — The Anthocyanin Powerhouse
Bilberry extract is a concentrated source of anthocyanins, powerful polyphenols known for their antioxidant and anti-inflammatory properties. In the context of longevity, it is studied for its ability to support vascular health, enhance microcirculation, and activate cellular repair pathways like AMPK and autophagy.
Mechanism of Action
Bilberry extract is rich in anthocyanins, particularly delphinidin and cyanidin glycosides, which act as potent antioxidants and signalling molecules. In preclinical models, bilberry anthocyanins reduce oxidative stress and activate autophagy via the AMPK/mTOR signalling pathway. They also modulate inflammatory cascades by inhibiting NF-κB activation and reducing the expression of pro-inflammatory cytokines. Additionally, these compounds may enhance endothelial nitric oxide synthase (eNOS) activity, improving vascular tone and microcirculation.
Human Trial Evidence
Human trials primarily focus on vascular health, eye health, and metabolic markers rather than lifespan extension. Several small clinical trials show that anthocyanin-rich bilberry extracts (160–480 mg/day) can improve microcirculation, reduce postprandial glycaemia, and lower systemic inflammation markers. No published human longevity trials exist; lifespan data is limited to animal and in-vitro models.

Typical doses range from 160 to 480 mg/day of standardised bilberry extract (often standardised to 25–36% anthocyanins). It is commonly taken in divided doses (e.g., 160 mg twice daily) with food to enhance absorption. Long-term safety is well-established at these doses for up to 6 months.
Safety & Contraindications
Bilberry extract is generally well tolerated at standard doses. High doses may theoretically increase bleeding risk due to its mild antiplatelet effects, so caution is advised for individuals on anticoagulant or antiplatelet medications (e.g., warfarin, aspirin). It may also have mild hypoglycaemic effects, requiring monitoring in patients taking antidiabetic drugs. Avoid excessive doses during pregnancy and lactation due to insufficient safety data.
