Bioperine — The Bioavailability Enhancer
Bioperine is a patented extract of piperine, the primary active alkaloid found in black pepper, widely used to enhance the bioavailability of poorly absorbed nutrients and supplements. In the context of longevity, it is rarely used as a standalone intervention but is considered essential for maximising the systemic absorption and efficacy of key molecules like curcumin and resveratrol.
Mechanism of Action
Bioperine (piperine) functions primarily as a bioenhancer by inhibiting key drug-metabolising enzymes in the liver and intestines. It potently inhibits cytochrome P450 3A4 (CYP3A4) and uridine diphosphate-glucuronosyltransferase (UGT) enzymes, which slows the glucuronidation and clearance of various compounds, allowing them to remain in systemic circulation longer. Additionally, piperine inhibits P-glycoprotein, an efflux pump that normally prevents the intracellular accumulation of certain drugs and nutrients. It also stimulates thermogenesis by activating transient receptor potential vanilloid 1 (TRPV1) receptors, which may further facilitate nutrient absorption in the gastrointestinal tract.
Human Trial Evidence
Bioperine has been extensively studied in humans primarily as an adjuvant to enhance the absorption of other compounds. The landmark 1998 study by Shoba et al. demonstrated that co-administering 20 mg of piperine with 2 g of curcumin increased curcumin bioavailability by 2000% in healthy human volunteers. It has also been shown to increase the absorption of iron, beta-carotene, and resveratrol in human trials. There are no published human longevity trials evaluating Bioperine as a standalone anti-ageing intervention.
Dosing Protocol
5–20 mg per day, typically taken simultaneously with the target compound (e.g., curcumin, resveratrol) to enhance absorption. The most common clinical dose is 5 mg per serving. It should be taken orally with water. Because it alters drug metabolism, timing should be carefully managed if taking prescription medications.
Safety & Contraindications
Bioperine is generally well-tolerated at standard doses (5–20 mg/day). However, because it potently inhibits CYP3A4 and P-glycoprotein, it can significantly alter the pharmacokinetics of many prescription medications, including statins, calcium channel blockers, and certain antidepressants, potentially leading to drug toxicity. It should be used with extreme caution or avoided in individuals taking medications with a narrow therapeutic index. High doses may cause gastrointestinal irritation.
