Citicoline (CDP-Choline) — The Neuroprotective Choline Source
Citicoline (cytidine diphosphocholine, CDP-choline) is a naturally occurring compound that serves as a precursor to both phosphatidylcholine and acetylcholine. It is one of the most bioavailable and neuroprotective choline sources, with strong evidence for cognitive function, stroke recovery, and age-related cognitive decline. Unlike choline bitartrate, citicoline also provides cytidine, which converts to uridine — a key component of neuronal membranes.
Mechanism of Action
Citicoline is hydrolysed to choline and cytidine in the gut. Choline is used for acetylcholine synthesis and phosphatidylcholine production (neuronal membrane integrity). Cytidine converts to uridine, which supports RNA synthesis and activates P2Y receptors involved in neuroprotection. Citicoline also increases dopamine receptor density and reduces glutamate excitotoxicity.
Human Trial Evidence
A 2012 Cochrane review of 14 RCTs found citicoline improved memory and behaviour in cognitive impairment. A 2015 Journal of Attention Disorders RCT showed 250–500 mg/day improved attention and psychomotor speed in healthy adults. Multiple stroke trials show reduced infarct volume and improved neurological outcomes with 500–2,000 mg/day.
Dosing Protocol
250–500 mg/day for cognitive enhancement in healthy adults. 500–2,000 mg/day for neurological conditions (under medical supervision). Best taken in the morning — may cause insomnia if taken late. Divide into two doses (morning and midday) at higher amounts. Effects on cognition typically noticed within 4–6 weeks.
Safety & Contraindications
Excellent safety profile. Well-tolerated in trials up to 2,000 mg/day. Mild GI effects possible. Headache reported rarely. No significant drug interactions at typical doses. Avoid late-day dosing to prevent insomnia.
