Coenzyme Q10 (ubiquinol) — The Mitochondrial Energy Cofactor

Coenzyme Q10 (CoQ10) is a fat-soluble compound essential for mitochondrial ATP production and cellular antioxidant defence. Endogenous synthesis declines significantly with age, and supplementation is widely used to support cardiovascular health, mitigate statin-induced myopathy, and preserve mitochondrial function.

Mechanism of Action

CoQ10 exists in two primary states: ubiquinone (oxidised) and ubiquinol (reduced). It functions as a mobile electron carrier in the mitochondrial electron transport chain, transferring electrons from Complexes I and II to Complex III to drive ATP synthesis. In its reduced form, ubiquinol acts as a potent lipophilic antioxidant, protecting membrane phospholipids and mitochondrial DNA from lipid peroxidation. It also influences gene expression related to inflammation and metabolism, potentially modulating NF-κB pathways.

Human Trial Evidence

CoQ10 has been extensively studied in human clinical trials, particularly for cardiovascular disease. The landmark Q-SYMBIO trial (2014, JACC: Heart Failure) demonstrated that long-term CoQ10 supplementation significantly reduced major adverse cardiovascular events and mortality in patients with chronic heart failure. Other trials have shown modest benefits in reducing statin-associated muscle symptoms and improving endothelial function, though evidence for direct lifespan extension in healthy humans remains unestablished.

Dosing Protocol

100–300 mg/day is the typical therapeutic range, often divided into multiple doses. Ubiquinol is generally preferred over ubiquinone due to superior bioavailability. Because it is highly lipophilic, it must be taken with a fat-containing meal to ensure adequate absorption.

Safety & Contraindications

CoQ10 is exceptionally well-tolerated, with a strong safety profile even at high doses (up to 1,200 mg/day). Mild gastrointestinal symptoms (nausea, upset stomach) are the most common adverse effects. It may interact with anticoagulant medications like warfarin by reducing their efficacy, and concurrent use should be monitored.
