Edaravone — The Neuroprotective Free Radical Scavenger
Edaravone is a potent antioxidant and free radical scavenger clinically approved for the treatment of amyotrophic lateral sclerosis (ALS) and ischemic stroke. While its primary application is neuroprotection in disease states, its ability to mitigate oxidative stress and lipid peroxidation makes it a molecule of interest in the context of cellular aging and neurodegeneration.
Mechanism of Action
Edaravone is a potent pyrazolone free radical scavenger and antioxidant. It directly neutralizes reactive oxygen species (ROS) and reactive nitrogen species (RNS), thereby inhibiting lipid peroxidation and preventing oxidative damage to cellular membranes and DNA. By mitigating oxidative stress, edaravone protects against mitochondrial dysfunction and neuronal apoptosis. Emerging evidence suggests it may also activate the Nrf2/HO-1 signaling pathway, further enhancing endogenous antioxidant defenses and reducing neuroinflammation.
Human Trial Evidence
No published human longevity trials. Human evidence is restricted to its approved use in amyotrophic lateral sclerosis (ALS) and acute ischemic stroke, where it has been shown to slow functional decline and improve survival outcomes in specific patient subsets. Animal and in-vitro evidence suggests broader cytoprotective and antioxidant effects, but these have not been evaluated for lifespan extension in healthy humans.
Dosing Protocol
For ALS, the standard FDA-approved dosing is 60 mg administered via intravenous infusion over 60 minutes daily for 14 days, followed by a 14-day drug-free period. Subsequent cycles consist of daily dosing for 10 out of 14 days, followed by 14 days off. An oral suspension (105 mg) is also available following the same cycle schedule. Edaravone is a prescription-only medication. Dose-finding for general longevity or anti-aging purposes is unestablished in humans.
Safety & Contraindications
Common adverse effects include contusion, gait disturbance, and headache. Intravenous administration carries risks of infusion site reactions. Edaravone contains sodium bisulfite, which may cause severe allergic reactions, including anaphylaxis, particularly in individuals with a history of asthma or sulfite allergy. Its safety in healthy individuals seeking longevity benefits is unknown, and it should be avoided outside of its approved clinical indications. Prescription required.
