Niacin (No-Flush) — Inositol Hexanicotinate
No-flush niacin, typically formulated as inositol hexanicotinate, is a form of vitamin B3 marketed to provide the benefits of niacin without the characteristic skin flush. While it serves as a source of vitamin B3, evidence suggests it is significantly less effective than standard nicotinic acid for lipid management and NAD+ elevation.
Mechanism of Action
Inositol hexanicotinate consists of six nicotinic acid molecules linked to an inositol molecule. It is hydrolysed in the body to slowly release free nicotinic acid and inositol. Because the release is gradual, it avoids the rapid spike in serum nicotinic acid that triggers prostaglandin D2-mediated vasodilation (the 'flush'). However, this slow release also means it fails to achieve the therapeutic blood concentrations required to significantly activate the HCA2 (GPR109A) receptor, which mediates many of niacin's lipid-modifying effects. Its conversion to NAD+ via the Preiss-Handler pathway is also limited by its pharmacokinetics.
Human Trial Evidence
Human trials evaluating inositol hexanicotinate for dyslipidemia have generally yielded disappointing results compared to immediate- or extended-release nicotinic acid. Studies indicate it does not significantly lower LDL cholesterol or triglycerides, nor does it meaningfully raise HDL cholesterol. There are no published human longevity trials specifically evaluating no-flush niacin for lifespan extension or anti-ageing endpoints.
Dosing Protocol
Commonly sold in doses of 400–800 mg per day. However, due to its lack of efficacy for lipid modification, it is not recommended as a substitute for prescription nicotinic acid. If used for general vitamin B3 supplementation, standard dietary doses (14–16 mg/day) are sufficient.
Safety & Contraindications
Generally well tolerated with a lower incidence of flushing, itching, and gastrointestinal distress compared to standard nicotinic acid. However, high doses of any niacin derivative carry a risk of hepatotoxicity, insulin resistance, and elevated uric acid levels. It should be used with caution in individuals with liver disease, gout, or diabetes.
