SGLT2 inhibitor (empagliflozin) — The Caloric Restriction Mimetic
Empagliflozin is an SGLT2 inhibitor originally developed for type 2 diabetes that has emerged as a potent cardioprotective and nephroprotective agent. By promoting glucose excretion and mimicking a state of fasting, it activates key longevity pathways like AMPK while inhibiting mTOR, making it a compelling candidate for extending healthspan even in non-diabetics.
Mechanism of Action
Empagliflozin primarily inhibits the sodium-glucose cotransporter 2 (SGLT2) in the proximal renal tubules, promoting glycosuria and lowering blood glucose. Beyond glycemic control, it acts as a caloric restriction mimetic by shifting systemic metabolism toward fatty acid oxidation and ketogenesis. At the molecular level, empagliflozin activates the energy-sensing kinase AMPK and sirtuin 1 (SIRT1), while suppressing the nutrient-sensing mTORC1 pathway. This metabolic reprogramming enhances autophagy, reduces oxidative stress, and mitigates systemic inflammation, contributing to its cardioprotective and nephroprotective effects.
Human Trial Evidence
Empagliflozin has robust human evidence for cardiovascular and renal protection, though not explicitly for lifespan extension in healthy individuals. The EMPA-REG OUTCOME trial (2015, NEJM) demonstrated a 38% reduction in cardiovascular mortality in type 2 diabetics. The EMPEROR-Preserved trial (2021, NEJM) showed significant reductions in cardiovascular death and heart failure hospitalizations in patients with preserved ejection fraction, independent of diabetes status.
Dosing Protocol
10–25 mg orally once daily. Prescription required. Best taken in the morning, with or without food. Dose adjustments may be necessary based on renal function (eGFR).
Safety & Contraindications
Prescription required. Common adverse effects include genital mycotic infections and urinary tract infections due to increased urinary glucose. Rare but serious risks include euglycemic diabetic ketoacidosis (DKA), volume depletion, and hypotension. Contraindicated in patients with severe renal impairment (eGFR < 20 mL/min/1.73 m²) or on dialysis. Caution is advised when combined with diuretics or insulin due to risks of hypovolemia and hypoglycemia.
