Shiitake (Lentinan) — The Immunomodulating β-Glucan

Lentinan is a high-molecular-weight β-glucan extracted from the shiitake mushroom (Lentinula edodes). It is widely studied for its immunomodulatory properties, particularly its ability to enhance macrophage and T-cell function, making it relevant for counteracting age-related immune senescence.

Mechanism of Action

Lentinan acts primarily as a biological response modifier. It binds to pattern recognition receptors such as Dectin-1, Toll-like receptor 2 (TLR2), and complement receptor 3 (CR3) on the surface of innate immune cells like macrophages, dendritic cells, and natural killer (NK) cells. This binding triggers intracellular signaling cascades, including the activation of NF-κB and MAPK pathways, leading to the production of cytokines such as IL-1, IL-12, TNF-α, and IFN-γ. These cytokines subsequently stimulate adaptive immune responses, enhancing T-cell proliferation and cytotoxic activity, which helps mitigate immune decline.

Human Trial Evidence

Lentinan has been extensively studied in human clinical trials, primarily as an intravenous adjuvant therapy for gastric and colorectal cancers, where it has been shown to improve survival rates and quality of life. However, there are no published human longevity trials evaluating lentinan for lifespan extension in healthy aging populations.

Dosing Protocol

In clinical settings for cancer adjuvant therapy, lentinan is typically administered intravenously at doses of 1–2 mg once or twice a week. For oral supplementation using shiitake mushroom extracts standardized for β-glucans, typical doses range from 1,000 to 3,000 mg per day. The optimal oral dose for general immune support and longevity remains unestablished.

Safety & Contraindications

Intravenous lentinan is generally well-tolerated but can cause mild adverse effects such as injection site reactions, mild fever, chills, and gastrointestinal disturbances. A rare but notable side effect is shiitake dermatitis, a whiplash-like rash caused by consuming raw or undercooked shiitake mushrooms. It should be used with caution in individuals with autoimmune diseases or those taking immunosuppressive medications.
